Biological approach in periodontal regeneration
The supportive treatment of infectious and chronic inflammatory diseases with implications on systemic health, such as periodontal diseases, is a medical act with high professional responsibilities. Through the application and integration of novel procedures and advanced technologies the general dentist is actually able to adequately manage most periodontal patients. This tendency towards simplification is widespread throughout all the branches of the medicine and derives from the need for a new medical model of appropriateness that make it possible to be at the same time efficacy and efficient. In periodontal regeneration the biotechnologies as well as the surgical procedures are increasingly oriented towards minimally invasive approaches exploiting the biological mechanisms of the wound healing process. The non-surgical periodontal therapy combined with the delivery of pharmacological agents or the application of molecules known to induce the periodontal regeneration has been demonstrated effective in the regenerative therapy of intrabony defects. In selected cases, whose anatomy is adequate, the minimally invasive surgery coupled with the use of the blood clot, stem cells or amelogenins make this approach more efficient, limiting the indication for more invasive procedures to a lower percentage of bony defects.
Prof. Sofia Aroca obtained a degree of Doctor in Dental Surgical Medicine from University of Budapest (Hungary) in 1985; post-graduate of dental biomaterials in 1991, Certificate of Oral Implantology in 1992 and post-graduate in Periodontology in 1996 from University Paris VII (France); and PhD in 2010 from University of Szeged Faculty of Dentistry (Hungary). She were Assistant Professor in 1998-2001 and Visiting Professor on Post-graduate in Periodontology and Oral Implantology since 2001 at the University Paris VII (France). She is Visiting Professor at University of Szeged Faculty of Dentistry (Hungary) since 2008 and Bern University, Department of Periodontology (Switzerland) since 2009. She is Honorary Professor at University of Szeged Faculty of Dentistry (Hungary) since 2012.
Increasing keratinized tissue around implants
Our clinical goal when restoring missing teeth is to provide functionality, stability and to improve esthetics. The soft tissue / implant interface consists of a tight band of circular fibers holding soft tissues close to the implant neck. The circumferential sealing effect given by dense connective and keratinized tissue is a prerequisite for the long term success of a dental implant. Scientific evidence shows that failure to achieve this seal might result in apical migration of epithelium down to the implant-bone interface. From a clinical stand-point a reduced or inexsistant keratinized mucosa around implants may favor: The mucosal inflammation due to the plaque accumulation, the risk for bone resorption, soft tissue dehiscence and also clinical attachment loss. We will discuss extensensively: How to manage properly the soft tissue around implants and which are the surgical techniques for keratinized tissue augmentation. The right time to apply one of these procedures is according to the clinical situation. I will share with you, based on the scientific literature, my clinical experience on this topic.

Porcelain veneers from diagnosis to luting
Contemporary esthetic dentistry aims to achieve long term restorations that are performed with minimal tooth preparation. In recent years due to their minimal invasive approach and successful clinical outcome, porcelain veneers have become the treatment of choice both for patients and dentists in esthetic dentistry in order to perform tooth alignment, color, shape, length and position modification, to close the diastemas, to restore fractured and endodontically treated teeth or to design a full smile. The esthetic and clinical success of porcelain veneers relies on the diagnosis, treatment planning, tooth preparation and adhesive luting procedures. Based on the scientific literature, this presentation will addresskey elements in diagnosis, precise treatment planning, tooth preparation in different cases and step by step luting procedures with up to date adhesive materials on various types of cases. The solutions to overcome the possible failures, such as postoperative sensitivity, marginal discoloration, fracture and debonding as well strategies to improve the durability of porcelain veneers will also be discussed.

Immediate dental implants: biology and clinical tips
Treatment planning at the anterior sites may be challenging due to esthetic requirements, and maintaining tissue contours is essential to provide lip support, as well as pink esthetics. Implant installation at the extraction site even requires more since primary stability as well as implant positioning further complicates the procedure. On top of all, placing a temporary restoration results in transmucosal healing and formation of biologic width. This presentation focuses on biology and clinical decision making in such conditions.

Supportive periodontal therapy around teeth and implants
To date, the world dental community has accumulated a significant positive experience in supporting periodontal treatment. The program of maintenance therapy should be made individually for each patient and depend on the nature and severity of local pathogenetic factors, the presence of background general somatic pathology, physiological characteristics of the body and the phase of the pathological process at which the treatment was first started. The report presents the rationale for determining the phasing and time interval during maintenance therapy around teeth and implants.

Stress & periodontal disease
Stress is a dynamic response of the organism against stressors (external and /or internal). It is an association of physiological and psychological reactions of a person confronted to a change of situation he cannot face. The effects of stress on our body are now well explained by the network existing between the neurologic, the immunologic, the psychologic and the endocrinal systems. The stress influence on the development and the progression of periodontal disease is demonstrated by a depressed immune system and by a change of behavior. The latest researchs are now more focused on the impact of patient’s « coping » wich is a good indicator for periodontal disease risk. Clinical cases will illustrate the theory. And finally, some advices we can give to our patients but that we also can apply in our own professional or private life, will be discussed.

Relationship between periodontal surgery and restorative dentistry for the esthetic reconstruction of multiple gingival recessions
Since patient aesthetic expectation and perception constantly increased in the last decades, the ultimate goal of mucogingival surgeries is not only the root coverage but also an aesthetic outcome, which results in a complete blending of tissue color and texture of the treated area with the adjacent soft tissues. The most widely utilized surgical technique are the coronal advanced flap and the multiple coronal advanced flap with or without connective tissue graft. One of the most frequent problem a clinician can encounter in performing mucogingival surgery is the absence of a cemento-enamel junction due to the presence of non-carious cervical lesion. The major concern related with the undetectability of the cemento-enamel junction is the difficulty to predetermine the maximum root coverage level. A method to predetermine is based on the calculation of the ideal height of the anatomic interdental papilla was demonstrated to be reliable in predicting the position of the soft tissue margin after root coverage surgery. Following this method, it is possible to recreate the cemento-enamel junction at the level of maximum root coverage, thus obtaining stabile and esthetic results.

Soft Tissue Augmentation Procedures Around Implants in Aesthetic Sites
Over the past 50 years, dental implants have evolved into a very straightforward treatment option for replacing missing or damaged teeth. But the ongoing development of implant therapy constantly reveals examples of how the quality and quantity of peri-implant soft tissues are the key elements when it comes to achieving top results. Furthermore, natural and harmonious results around dental implants located in the esthetic zone are calls for preservation and sometimes the reconstruction of the soft tissue. Effective surgical techniques for soft tissue management around implants in the esthetic zone have been developed and should be implemented in every clinician’s treatment spectrum. The need for augmentation should be analyzed before every intervention and it may also be necessary to augment soft tissue at multiple steps during the course of implant therapy so that the procedures are subsequently combined. This lecture will be present how to create or preserve soft tissue around implants in esthetic zone; from analyzing to reconstruction of the soft tissue.
